Fix: 1. Background image of dungeon thumbnail is corrupt in dungeon selection screen when dungeon is too big (20*20). 2. Clear single equipment slot inside dungeon cause error. 3. BGM changes too frequently during tutorial.

1. Add Death Panel in case player don't know what happened after character death. Player can check "Do Not Show Again" to avoid interruption in future, also can enable it again in option panel. 2. Change unavailable game features in tutorial from totally invisible to transparent.
