Mark had to enter the hospital due to a mental illness(we can see this being the reason due to it being stated in a divorce decree). While Mark spent years in the hospital, Sara decided that the best thing to do was starting a new life far away from Mark and therefore boarded the Shakhter-One.
Shakhter-One is a space rocket ship dedicated to minery. It was built by russians using cheap sources of labour and gain benefits from it as fast as possible. This ship landed in Jupiter to start the extraction of the minerals, and, unluckily, one of the planet's compound was unknown and had a chemical composition close to the one heroine has. The systems were unable to purify this compound and before the crew could notice, they were being infected.
When Mark exit the hospital , following Sara, he boarded the Shakhter-One aswell. In this ship, he worked as a pilot and as when the virus spread across the crew he got infected. Sara, despite of everything she suffered when they were married, helped him to escape so that Mark wouldn’t have to face the same destiny the infected ones had to.
Mark, due to one of the virus symptoms, forgot about what happened and came back to the ship crashing against the Shakhter-One in the endless search of Sara (we can see the crashed ship before fall down in Cables' maze).
Mark was being chased by the Shakhter-One tripulation so, he decided to hide himself on the Cables' maze and wait for the situation to calm down. While this was happening, Sara was enclosed for helped Mark to escape from Shakhter-One.
Whole the situation makes Sara fall ill and for this reason the tripulation thought that Sara was infected and they enclosed her in an isolation cell with the infected ones (for this reason Sara sometimes says: ''I wasn't infected, you were the infected one, Mark''). Sara couldn't resist anymore so she decided to kill herself because she knew that Mark never would stop until recover Sara and she dindn't want to stay with him anymore, she wanted to start a new life.
Mark was still hiding in the cables' maze therefore finally he died because of the UC. It is a virus which makes people become crazy and see things that are not really there. For this reason they make things like suicide, homicide and cannibalism.
The UC can kill your body but it can not kill your souls but it can harrow it. So, the ones who get killed by the UC, are doomed to wander for their darkest and deeper thoughts. For this reason at the start of the game Mark says: ''I don't know who I am or even if this happened before. If this hapenned before, I could remember it, right?''
When we arrive at the cables' maze, we can discover that our guide in the game, Mike, is really ourself in another time. He said: ''We have tried to do this another times but the UC always reached us so we had to start again''.
This is what we exactly see at the final A of the game. We destroy the Shakhter-One but this is not real, the really Shakhter-One is wandering near Jupiter (that is the unknown planet which we can see when the reaper reached us) and the real Mark is dead in the cables' maze. So the UC reset our mind and we have to start the same story again.
In the case of Mark, he is doomed to wander in the search of Sara for his eternity because he couldn't let Sara go in the real life and his soul can not do it so, he will be living the same torture again and again. For this reason at the final B of the game we can hear Sara saying: ''I couldn't resist anymore so I killed myself Mark. You have to stop this, I am dead and you can not make me your demon for the rest of the eternity''.
Therefore, Mark's soul is trapped in an infinite loop where he is trying to recover Sara and repair his errors in the past and where he is close to the nightmare's end the UC just reset his mind and he has to start again.
