- We have prepared a complex new psychedelic weapon
- A new delightfully evil multi-stage boss has been unleashed
- New optional miniboss battles have been tucked away in hidden locations
- Two new difficulty settings have been added
- Easy mode is an easier experience of the hard boss content from The Final Specimen with all of the new material also included
- Hard mode is the toughest challenge yet with brand new sinister twists added to all 8 bosses
- There are now multiple possible game endings
- We have cursed you with several chilling cinematic gameplay moments
- You can now experience true shock and horror when you lose a continue
- There is a slamming new equine title screen
- We have included a handful of new achievements

- Subtitles and interface support has been added for Italian, Arabic, Czech, Dutch, Greek, Hungarian, Japanese, Norwegian, Polish, Romanian, Swedish, Thai, Turkish, and Ukrainian
