There is a bug in hospital when depending of your fps item can fall through the floor.
And the Alien can kill you through wall.
The patch to fix these is currently compiling so it should be fixed in a hour maximum.
(Also added a safety in case it already happened to you the item will respawn so your save won't be unusable)
