- Randomiser Mode: The Clone anomaly could cause a crash if the special Flower xenos were spawning.
- Sector 2: If a prisoner was captured by the train anomaly then it would cause a crash.
- Sector 2: The Apple train could capture prisoners.
- Sector 2: If a prisoner was freed from a Xenos that was over a train track junction then the prisoner could move along the track.
- Level Room - Prisoner Lift: Using a Repair action on it wasn't re-sealing its bottom doorway.
