Issue #1

You shoot someone right in the face, expecting them to keel over and die, only for them to turn around and shoot you. You thought you shot them in the face, but you actually shot them in the torso.

But why does this happen?
Let's take a look at the MiB's collision from above:

So where is the head defined for a headshot?

You might expect the area defined as the head to be here, right where the head is. Or even here, right in the middle of the collision. But the correct answer is this cross.

Due to your shot always landing at the edge of the circle (unless you shoot them from above) even if you shoot where their head is visually you'll end up hitting the torso if shooting from a diagonal. You always had to hit front/sides/back directly to count as a headshot.

Issue #2

You aim right at someone's head, you pull the trigger annnnnnnd... nothing.
They take no damage, your shot somehow went through their head and still missed.

Why does this happen? Look at their collision box:

The top of their head has no collision, no-one's forehead has collision, this applies to every person in the game.
This was done by the developers to fix issues with models floating.
With female characters it's even worse, nothing above their eyes has collision:

Fixing these Issues

Regarding issue #1, the next patch will have all headshots in the head area do head damage, regardless of angle. Unless I get some help to make headshots even more accurate and only hit the visual area of the head.

Regarding issue #2, in the next patch all characters will have their collision increased in order to give their foreheads collision.

The side effects of this are obvious, the game will become easier due to the more reliable headshots, but when the previous difficulty came from hidden jank, I believe this is fair.
The other side effect from increasing the collision is that people have become an inch or two taller, JC will no longer look down on people as much.
