Fixed the HUD mode icon forced on if a controller is connected. The icon is now "controller-only" by default, and entering hud_mode 0 into the console will force it off, whereas hud_mode 2 will force it on whether a controller is connected or not.
Fixed the HUD mode icon appearing before the player obtains the HEV suit.
