Fixed all reported bugs, most notably ones that blocked the player such as the one in the Machine Factory optional dungeon.

Fixed a couple reported bugs. - Andromeda optional boss crashing in its opening attack sequence. - Could enter the Revolution Ruins too early in the story. - Database entry was not updated to match some skills.
